The Office of Lead Hazard Control and Healthy Homes has received your application for a
grant under the Fiscal Year 2021 Lead Hazard Reduction Grant Program, Catalog of Federal
Domestic Assistance (CFDA) number 14.905. The Notice of Funding Opportunity (NOFO) advised
prospective applicants of grant eligibility, submission, and threshold requirements that need to be
met in order for the application to be accepted and considered for review. In addition, Section
IV.D.4 (Application and Submission Information) of the General Section of the NOFO (issued
August 27, 2021) described how corrections to applications with correctable deficiencies are to be
handled (see page 34).
During preliminary review of your application one or more of the following correctable
deficiencies were identified:
1. The form HUD 2991 was not submitted with your application. Please submit Form 2991.
Please review the deficiencies identified above and submit the corrections via e-mail to
[email protected] To ensure that your application can be accepted for review,
corrections of the deficiencies noted must be received by HUD within 10 calendar days of the
date of receipt of this notification. If your emailed response is not received within the 5-day
period, your application will be deemed ineligible and removed from consideration.
The email subject line should include the following header information: applicant legal
name, and application number. The text should provide the Catalog of Federal Domestic
Assistance (CFDA) number the applicant’s legal name, city, and state and a list of attachments.
This ensures that your response will be included with your application for a complete evaluation.
