Minor Revisions to the Collection Instruments

SSA is making the following revisions:

• Change #1: We are revising the PRA statements for the following forms under this information collection: SSA-4659, SSA-85, SSA-2935, SSA-8552, SSA-L8553, and SSA‑L8554.

Justification #1: We are revising the PRA statements to reflect our current boilerplate language.  The current language, which dates back to the last reprint of the form, is now outdated.

Note: We have included a sample of the updated language we will use for the PRA Statements. We will ensure that each individual forms shows the appropriate burden times for that form in the PRA Statement.

• Change #2: We are revising the Privacy Act Statements for the following forms under this information collection: SSA-4659, SSA-85, SSA-2935, SSA-8552.

Justification #2: SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statements on this collection.
