willfully fail to file or refuse to register or be fingerprinted, make any
misrepresentation or attempt to procure registration through fraud, falsify
or conceal a material fact, or submit a false information on your G-325R,
Biographic Information (Registration) you may be fined or imprisoned as
provided in Title 18 of the U.S. Code and 8 U.S.C. 1306.

I understand that USCIS will require me to appear for an appointment to take my biometrics
(fingerprints, photograph, and/or signature) and, at that time, I will be required to sign an
oath reaffirming that:
1. I reviewed and provided or authorized all of the information in my ${formType} ;

and
