The Telecommunications Act of 1996 provides that discounts on eligible services and equipment will be provided to eligible schools and libraries, as well as consortia of schools and libraries. FCC Forms 472, 473, and 474 facilitate the reimbursement process by allowing applicants and service providers to request reimbursement of the discounted costs of services and equipment provided to eligible entities and provide certifications regarding receipt of the services and equipment and other program requirements.

The Commission is reporting a program change and an adjustment to the total burden hours within this submission. The adjustment is due to the estimated time per response increasing by 0.5 hours for FCC Form 473, updated information on actual submissions of the FCC Form 472, FCC Form 473, and FCC Form 474, and the change in number of respondents. For FCC Form 472: the number of respondents decreased by 1,085 from 9,500 to 8,415; responses decreased by 3,255 from 28,500 to 25,245; and annual burden hours decreased by 4,882 from 42,750 to 37,868. For FCC Form 473: the estimated time per response increased by 0.5 hours from 1.5 hours to 2 hours (the increase is due to the data usage reports and non-usage notices that service providers are required to provide for Wi-Fi hotspots and services used off-premises); the number of respondents and responses decreased by 458 from 5,000 to 4,542; and the annual burden hours increased by 1,584 from 7,500 to 9,084. For FCC Form 474: the number of respondents decreased by 61 from 2,100 to 2,039; the number of responses increased by 27,470 from 63,000 to 90,470; and the annual burden hours increased by 41,206 from 94,500 to 135,706. Overall, for the adjustments, the number of respondents increased by 11,353 from 16,600 to 27,953, the number of responses increased by 36,714 from 96,500 to 133,214, and the total annual burden increased by 50,865 from 144,750 to 195,615. The program changes are due to the certifications being added to FCC Forms 472, 473, and 474. There are no changes to the burden hours for completing FCC Forms 472, 473, and 474, since the program changes only include adding additional certifications that applicants will check off.
