INTRODUCTION Thank you for taking the time to complete this post-workshop survey! This survey supports ongoing evaluation of CDC’s One Health Zoonotic Disease Prioritization (OHZDP) Process to measure the impact of the OHZDP around the world. The purpose of this survey is to learn how locations have advanced One Health efforts since engaging in the OHZDP Process.
This survey will be issued at three time points (6 months, 1.5 years, and 3 years after your OHZDP workshop). After completing each survey, you will be able to track your location’s progress to advance One Health and address the priority zoonotic diseases since the OHZDP workshop.
Information provided may be used by CDC for monitoring and evaluation. Responses may be aggregated with other locations in a summary report on OHZDP impacts globally.
