Subject: Request for Feedback: A Survey About CDC’s Clinical Guidance on Heat and Health

Hello,

NACHC is working with the Centers for Disease Control and Prevention (CDC) and the National Center for Farmworker Health (NCFH) to conduct a survey seeking your feedback on the CDC’s recently released clinical guidance on heat and health for health centers (CHC).

The purpose of this data collection is to understand health centers’ familiarity and use of CDC’s heat and health guidance and to assess the usability and content of the guidance materials. Even if your CHC has not implemented the guidance materials, we would appreciate your feedback.

We are inviting the person in your CHC who is most likely to counsel CHC patients on heat protective measures to complete this survey. That may be you or may be a clinician or other non-clinical staff in your CHC. If you are counseling patients in your CHC on heat protective measures, please complete this survey. If you are not counseling patients in your CHC on heat protective measures, or you are counseling patients and know that others in your CHC are also counseling patients, please forward this survey invitation to other patient services staff who may have occasion to use tools that help screen patients for heat exposure or provide clinical guidance and/or resources to patients related to extreme heat, whether they have used the CDC heat and health clinical guidance and tools or not. This may include other clinicians, community health workers, patient educators, health and safety team members, patient navigators, and/or call center staff. Multiple staff members from a single health center may complete the survey. Participation in the survey is voluntary.

Attached is a flyer about the survey opportunity. We encourage your health center to print and post in appropriate areas to reach staff, for example in clinician and staff break rooms and other common areas for staff only.

CDC is offering a $30 Visa e-gift card as a token of appreciation for each individual completing the survey. The survey should take about 15 minutes to complete.

Your survey response and those of your staff will help CDC and partners continue to support patients with needed resources addressing extreme heat.
