RE: OMB Control No. 2900-0222, VA Form 40-1330, Claim for Standard Government
Headstone or Marker, and VA Form 40-1330M, Claim for Government Medallion for
Placement in a Private Cemetery.
Dear Mr. Fletcher,
The Department of Veterans Affairs (VA) National Cemetery Administration (NCA)
appreciates the comment submitted on behalf of the Veterans of Foreign Wars of the
United States’ (VFW’s) in response to the renewal notice for the 2900-0222 information
collection.
NCA agrees that changing the title in Block 16 from “War Service” to “Periods of War”
would more accurately reflect service during a period of war, as defined by 38 CFR
§ 3.2. We believe this clarification on the form will help applicants provide information
about a decedent’s wartime service that NCA will verify as part of its development for
eligibility and inscription accuracy purposes. Enclosed is a copy of the revised VA Form
40-1330 which shows the change Block 16 to reflect “Periods of War”.
Again, we thank the VFW for the comment on this collection of information and support
of our Nation’s Veterans.

Respectfully,
