DOMESTIC URANIUM PRODUCTION REPORT (QUARTERLY)
YOUR RESPONSE IS REQUIRED BY LAW
This report is mandatory under Title 15 U.S.C. §772(b). Failure to comply may result in criminal fines, civil penalties and other
sanctions as provided by Title 15 U.S.C. §797. Title 18 U.S.C. §1001 makes it a criminal offense for any person knowingly and
willingly to make to any Agency or Department of the United States any false, fictitious, or fraudulent statements as to any
matter within its jurisdiction.
PURPOSE
The Form EIA-851Q “Domestic Uranium Production Report (Quarterly)” is used to collect data about the U.S. uranium industry
on monthly uranium production and sources (mines and other). Firms and individuals that are involved in the (domestic) U.S.
uranium industry must complete the Form EIA-851Q. It is completed by uranium concentrate producers and firms with existing
or planned facilities: uranium mills, uranium in-situ recovery plants, uranium by-product plants, and uranium heap leach plants.
The data collected on this form may appear in the following U.S. Energy Information Administration (EIA) publications: Domestic
Uranium Production Report – Annual and Domestic Uranium Production Report - Quarterly.
RESPONSE DUE DATE
The Form EIA-851Q should be submitted to EIA within 15 days after the end of the reporting quarter.
The reporting quarters are: First Quarter, January 1 – March 31; Second Quarter, April 1 – June 30; Third Quarter, July 1 –
September 30; Fourth Quarter, October 1 – December 31. The respective due dates are April 15, July 15, October 15, and
January 15.
HOW TO FILE A RESPONSE
To facilitate the processing of data, the use of EIA forms is required. The form can be downloaded in XLS format on the EIA
website, which can be accessed from http://www.eia.gov/survey/#eia-851q.
Survey respondents can submit data electronically using EIA’s secure Single Sign-On internet data collection system. This system
uses security protocols to protect information against unauthorized access during transmission. If you have not registered with
EIA’s Single Sign-On system, send an e-mail requesting assistance to [email protected] . If you have registered with Single SignOn, log on at https://signon.eia.doe.gov/ssoserver/login . If you are having a technical problem with accessing or using the
Single Sign-On system, send an e-mail requesting assistance to [email protected] .
Respondents can also use the EIA’s Secure File Transfer system to submit their data. With this Internet-based option, EIA uses
security protocols to protect the information against unauthorized access during transmission. EIA does not accept email, fax,
or paper forms.

HOW TO USE EIA’S SECURE FILE TRANSFER
EIA is ensuring the security of your transactions by using the latest Internet security technology. The technology being used to
protect your data is encryption which is the scrambling of data into a code that is unreadable to anyone who does not have the
key that deciphers it. The secure hypertext transfer protocol (HTTPS) is a communications protocol designed to transfer this
encrypted information between computers over the internet. All information is protected by 128-bit encryption to maintain the
privacy and confidentiality of your data. The only thing you need to take advantage of strong encryption technology is a secure
browser, one that supports 128-bit encryption.

Go to the EIA Secure File Transfer system located at https://signon.eia.doe.gov/upload/noticeoog.jsp

Read the Agreement and then click the Accept button.

Enter your name, company name, phone number and email address into the boxes provided. Note that the email
address is required so that we can send you a confirmation of the receipt of your data.

Click on the Choose Files button to navigate to your saved Excel file submission. Select the file to upload and click on
the Open button.

If you are ready to submit your file, click on the green Submit File(s) button. Please be patient, it may take a few
minutes to upload your file. Do not close your browser during this upload. A confirmation page will be displayed with
a Submission Successful banner and indicate the names of the files you have transferred, a confirmation number and
the date and time of the transfer.
SANCTIONS
The timely submission of Form EIA-851Q by those required to report is mandatory under 15 U.S.C. §772 (b). In accordance with
15 U.S.C. §797, failure to respond may result in a civil penalty of not more than $12,937 each day for each violation. The
government may bring a civil action to prohibit reporting violations, which may result in a temporary restraining order or a
preliminary or permanent injunction without bond. In such civil action, the court may also issue mandatory injunctions
commanding any person to comply with these reporting requirements.
REPORTING BURDEN
Public reporting burden for this collection of information is estimated to average 1 hour per response, including the time for
reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and
reviewing the collection of information. Send comments regarding this burden estimate or any other aspect of this collection of
information, including suggestions for reducing this burden, to the U.S. Energy Information Administration, Survey Development
and Statistical Integration, EI-21, Forrestal Building, 1000 Independence Avenue S.W., Washington, D.C. 20585-0670; and to the
Office of Information and Regulatory Affairs, Office of Management and Budget, Washington, D.C. 20503. A person is not

DISCLOSURE OF INFORMATION
The ‘Respondent Identification’ (Company Name), ‘Item 1: Facility Information’, and production data reported on Form EIA851Q are considered public information and may be released in company identifiable form. Additional information reported on
this form may be protected and may not be disclosed to the public to the extent that it satisfies the criteria for exemption under
the Freedom of Information Act (FOIA), 5 U.S.C. 552, the Department of Energy (DOE) regulations, 10 CFR 1004, implementing
the FOIA, and the Trade Secrets Act, 18 U.S.C. 1905. The Federal Energy Administration Act requires EIA to provide companyspecific data to other Federal agencies when requested for official use.
The information reported on this form may also be made available, upon request, to another component of the Department of
Energy (DOE); to any Committee of Congress, the Government Accountability Office, or other Federal agencies authorized by
law to receive such information. A court of competent jurisdiction may obtain this information in response to an order.
The information may be used for any non-statistical purposes such as administrative, regulatory, law enforcement, or
adjudicatory purposes. Data protection methods are applied to the statistical information reported on Forms EIA-851Q, except
for production data.
SPECIFIC INSTRUCTIONS
ITEM 1: FACILITY INFORMATION
Provide information about the uranium concentrate mill/processing facility. Rated capacity is synonymous with nominal capacity
and nameplate capacity. Indicate operating status at end of the Survey Quarter. Report rated capacity in short tons of ore per
day for conventional mills and heap leach facilities. For in-situ recovery and uranium by-product facilities, report rated capacity
in pounds U3O8 per year.
ITEM 2: PRODUCTION
For the Survey Quarter, provide the uranium production in pounds U3O8 for each of the 3 months.
ITEM 3: PRODUCTION SOURCE
Indicate where the Survey Quarter production in Item 2 came from (mines and other sources, i.e. alternate feed, byproduct, etc.).
First specify the mine source by entering each mine name and pounds U3O8 produced. Second, specify the other (non-mine)
source by entering the type of source and pounds U3O8 produced.
