Sample Guide for Semi-Structured Interviews

This interview guide includes core domains that are intended to capture i) characteristics of health care providers (physicians, nurse practitioners, and physician assistants), ii) implementation processes related to the AHRQ Safety Program for HAI Prevention, iii) barriers and facilitators related to the AHRQ Safety Program for HAI Prevention, iv) use and perceptions of the AHRQ Safety Program for HAI Prevention technical assistance and toolkits/materials, v) experiences with measurement during the AHRQ Safety Program for HAI Prevention, and vi) feedback about the AHRQ Safety Program for HAI Prevention. This guide will be refined to reflect differences between health care providers and key support staff. It will also be refined to reflect differences in settings. Finally, this guide can be amended for length and comprehensibility of questions. All interviews will occur at the end of the intervention period and last up to 30 minutes.

  • What year did you start practicing independently (post-training, i.e., residency and/or fellowship)?

  • Did your unit/facility have any HAI prevention efforts underway prior to implementation of the AHRQ Safety Program for HAI Prevention? If yes, what were they?

  • How ready do you think your unit/facility was to implement the AHRQ Safety Program for HAI Prevention?

  • Did you think it was necessary for your unit/facility to implement this intervention? Please explain.

  • Are there other patient safety and quality improvement initiatives going on in your unit/facility? If so, what are they?

  • How much leadership support do you feel that you have for improving patient safety and quality?
