Recruitment and Consent Scripts (English)
My name is _________________. We are calling you today on behalf of the CDC, the Centers for Disease Control and Prevention. I would like to invite you to participate in an upcoming discussion about heat stress-related issues at the workplace and educational products that could be used to prevent heat-related illnesses, like heat stroke and heat exhaustion at the workplace. If qualified, you will be invited to participate in a 60 to 90-minute focus group discussion. For your participation, you will receive a monetary gift of $40 incentive for participating. Would you be interested in participating in this study?
Thank you for agreeing to participate in this research study on heat stress that is being conducted by the Centers for Disease Control and Prevention (CDC), National Institute for Occupational Safety and Health (NIOSH). Your participation in the research is voluntary. I will ask you some questions about your opinions about heat stress-related materials and tools that could be used at your job which takes about 90 minutes to answer. You will receive $40 today as a token of appreciation for your participation. Your responses will be kept private (confidential) to the extent permitted by law and you can stop participating at any time. There is a small risk that your participation or the information you provide us will not remain confidential. To avoid loss of confidentiality, we will keep your information secure at all times. There is no direct benefit to you, except that your help may improve heat stress awareness and understanding for outdoor workers and their employers. There will be no costs to you, other than your time involved. If you have any questions or concerns regarding this study, you may contact Brenda Jacklitsch at 513-533-8369. Do you have any questions? Let’s begin.
My name is _________________. We are calling you today on behalf of the CDC, the Centers for Disease Control and Prevention, National Institute for Occupational Safety and Health (NIOSH). I would like to invite you to participate in an upcoming evaluation of a draft online heat stress training for employers of outdoor workers. Qualifying study participants will be sent a link to a pre-test via email prior to being sent a link to the online training. After viewing the online training, participants will be invited to participate in a post-test. About six weeks later, participants will be sent a link to a second post-test. This second post-test will be used to determine how much heat stress information you retained from the online training. In both pre‐ and post‐tests, there will be questions asking participants about heat-related illnesses and using heat stress prevention recommendations. In total, it will take approximately 60 minutes to participate. For your participation, you will receive $40 as a token of appreciation for your participation. Would you be interested in participating in this study?

Me llamo _________________.
Vamos a empezar.
