3 9/26/2025 Indiana My name is Kristen Bruss and I am the supervising attorney for the Lake County IV-D Child Support Office located in Gary, Indiana. I have been a deputy prosecutor with IV-D for 19 years and handle our UIFSA cases. I have reviewed the forms and make the following recommendations:

(1) Document 1, page 1. Include the date employment started. To me, that is more valuable then when it was verified.

(2) Document 1, page 2. Where it lists date of marriage, add location of marriage in case we need to verify.

(3) Document 1 instructions, add date employment began in addition to date of verification.

(4) Document 4, page 3, ask for day care verification paperwork at this point. I realize it is on the list of documents to supply, but it rarely happens.

(5) Document 4, page 9, Add a line for VA benefits.

(6) Document 5, section 1, add amount ordered to arrears, I end up writing this on most registration packets.

(7) Document 10, page 2. Have a check box for most preferred method of contact for the caseworker.
Thank you for the comment. We have not made any changes due to the following:

(1) Wage withholding can only be initiated after employment is verified, regardless of when employment began.

(2) If an applicant alleges a child was conceived during a marriage, paternity is already established, and the father’s name will appear on the child’s birth certificate. Additionally, location of marriage can be sought from the parties on the case. OCSS will consider this addition during the next PRA cycle.

(3) Wage withholding can only be initiated after employment is verified, regardless of when employment began.

(4) Day care verification paperwork should be communicated between the states. OCSS will reiterate to states, they must provide day care verification paperwork when submitting this form. This is not a suggested edit for the form.

(5) Page 9 section (n) allows states to include all other sources of financial information such as VA benefits that are not specifically listed on the form. OCSS will consider this addition during the next PRA cycle.

(6) Most arrears are accumulated after a child support order is established and therefore do not have a court ordered amount included in the child support order. However, OCSS will consider this addition during the next PRA cycle.

(7) This is not a necessary addition. All contact methods are provided on the form.
