The United States Department of Agriculture (USDA) recently mailed you a questionnaire for
the 2027 Census of Agriculture. Your response to the Census of Agriculture is required and
protected by law (Title 7 USC 2204(g) Public Law 105-113). If you are no longer involved in any
agricultural operations, please call us toll free at 888-424-7828 so we can update our records.
If you have already responded, thank you! If not, please respond by February xx, 20xx, in one of
the following ways:

Online at www.agcounts.usda.gov and enter your unique survey code found on the
front page of the Census of Horticultural Specialties questionnaire mailed to you.
Mail your completed questionnaire back using the prepaid envelope.
Fax your completed questionnaire to 812-218-2232.
