If the Medication Reconciliation measure is removed from the ESRD QIP, the Medication Reconciliation section in EQRS would need to be removed. This section is found within the “Manage Clinical” section of the Patient Registry. To access it, users select “Patients”, then “Manage Clinical”. The Medication Reconciliation section is found below the Ultrafiltration section and collects the following MedRec information:
