Request for ATF Background Investigation Information

Other Federal, state and local agency representatives requesting ATF background investigation information, must complete the Request for ATF Background Investigation Information - ATF Form 8620.65, as an official request for the information. ATF will make an authorized disclosure determination based on the type of agency requesting the information and the reason for the request. ATF will maintain the completed form as an official record of the request for information from the other agency.
