Description of Agency Usage:
This form must be submitted quarterly based on the following time periods: January 1 – March 31, April 1 – June 30, July 1 – September 30, October 1 – December 31. Quarterly reports are due within 30 calendar days of the end of the reporting period. A final report is due within 90 calendar days of the completion of the project or the end of the period of performance, whichever is comes first.
