SECTION 2 OF THE RAILROAD RETIREMENT
ACT PROVIDES FOR PAYMENT OF DISABILITY ANNUITIES TO QUALIFIED
RAILROAD EMPLOYEES. THE SUPPLEMENT TO THE EMPLOYEE'S APPLICATION
WILL OBTAIN INFORMATION ABOUT THE APPLICANT'S DISABILITY, DOCTOR OR
HOSPITAL EXAMINATIONS, EMPLOYMENT AND WORKMEN'S COMPENSATION. THE
INFORMATION WILL BE USED TO OBTAIN MEDICAL RECORDS AND STATEMENTS
NEEDED TO SUPPORT THE APPLICATION FOR THE DISABILITY
BENEFITS
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
