Abstract: SECTIONS 216 AND 223 OF THE SOCIAL SECURITY ACT PROVIDE FOR ESTABLISHING ENTITLEMENT TO DISABILITY BENEFITS. THIS FORM IS USED IN DETERMINING THE NATURE AND SEVERITY OF THE CLAIMANT'S ALLEGED IMPAIRMENT AND THE EFFECT ON HIS/HER ABILITY TO WORK. INFORMATION CONCERNING THE CLAIMANT'S SOURCES OF MEDICAL TREATMENT IS USED TO OBTAIN THE REQUIRED MEDICAL DOCUMENTATION.
