Abstract: THIS FORM IS USED TO OBTAIN INFORMATION ON THE TRAINING AND EXPERIENCE OF A NURSE APPLICANT TO DETERMINE HIS/HER SUITABILITY FOR EMPLOYMENT IN PHS FACILITIES AND THE PERSONNEL SYSTEM TO USED. IT ALSO PROVIDES INFORMATION NEEDED TO MATCH APPLICANTS' PREFERENCES WITH NURSING POSITIONS IN THE BUREAU OF MEDICAL SERVICES.
