Abstract: THE FORM IS NECESSARY TO CERTIFY THAT CERTAIN ANNUITANTS OR SURVIVOR ANNUITANTS ARE ELIGIBLE FOR A GOVERNMENT CONTRIBUTION TOWARD THEIR PREMIUM FOR PART B OF MEDICARE AS PROVIDED BY P.L. 91-418.
