Abstract: SECTION 233 OF PUBLIC LAW 95-216 (INTERNATION AGREEMENTS WITH REPECT TO SOCIAL SECURITY BENEFITS) PROVIDES FOR INFORMATION REQUIRED BY PERSONS FILING FOR SOCIAL BENEFITS UNDER THE AGREEMENT WITH ITALY. THIS FORM IS USED TO OBTAIN SUFFICIENT INFORMATION TO ENABLE BOTH ITALY AND THE U.S. TO DETERMINE WHETHER ENTITLEMENT TO BENEFITS EXISTS.
