Abstract: THE SURVEY IS A MAILOUT QUESTIONNAIRE TO ALL FULL YEAR AND SUMMER HEAD START PROGRAMS TO BE COMPLETED ON AN ANNUAL BASIS TO PROVIDE INFORMATION FOR THE ANNUAL REPORT TO CONGRESS REQUIRED BY P.L. 95-568 ON THE NUMBER OF HANDICAPPED CHILDREN ENROLLED, THEIR HANDICAPPING CONDITIONS AND THE SERVICES BEING PROVIDED.
