A WINDFALL BENEFIT AS A WIDOWER OR HUSBAND OF AN ANNUITANT, THE
APPLICANT MUST SUBMIT PROOF OF RECEIVING ONE-HALF OF HIS OR HER
SUPPORT FROM THE EMPLOYEE AT THE TIME OF THE EMPLOYEE'S DEATH OR
RETIREMENT. THE STATEMENT WILL OBTAIN INFORMATION USED TO DETERMINE
IF THE APPLICANT MEETS THE REQUIREMENTS FOR ENTITLEMENT TO THE
BENEFITS
