Abstract: THIS FORM IS REQUIRED FOR THE IMPLEMENTATION OF SECTION 210 OF THE EOA AS AMENDED, PUBLIC LAW #88-452 WHICH GIVES AUTHORITY TO THE LOCAL GOVERNMENTS TO DESIGNATE A COMMUNITY ACTION AGENCY. THIS FORM IS USED TO APPLY FOR RECOGNITION OF A CAA TO CSA BY THE DESIGNATING GOVERNMENT GOVERNMENT.
