Abstract: THIS FORM WILL BE USED IN CONJUNCTION WITH THE SUBMISSION OF AN APPLICATION FOR A LOAN UNDER THE PUBLIC WORKS AND ECONOMIC DEVELOPMENT ACT OF 1965. THE FORM WILL SPECIFY THE BORROWER'S ASSETS AND THEIR MONETARY VALUE, DURING LOAN PROCESSING AND LOAN CLOSING FOR WITH THE BORROWER CLAIMS CREDIT. THE FORM WILL ALSO BE USED BY EDA PERSONNEL TO EVALUATE THE BORROWER'S CLAIM.
