Abstract: SECTION 1611(C)(1) OF THE SOCIAL SECURITY ACT PROVIDES FOR INFORMATION REGARDING THE REDETERMINATION PROCESS FOR SUPPLEMENTAL SECURITY INCOME (SSI) BENEFITS. THIS FORM IS USED TO ELICIT MINIMAL INFORMATION FROM IS USED TO ELICIT MINIMAL INFORMATION FROM CAREFULLY SELECTED SSI RECIPIENTS TO PERMIT DISTRICT OFFICE PERSONNEL TO MAKE A DECISION ON CONTINUING ELIGIBILITY OF RECIPIENTS OR THAT FURTHER DEVELOPMENT IS NECESSARY
