Abstract: REQUIREMENT FOR THIS FORM IS FROM DISASTER RELIEF ACT '74, PL 93-288 & FED. DISASTER ASSISTANCE REGULATIONS (44 CFR 205). FOLLOWING EMERGENCY/MAJOR DISASTER DECLARATION BY PRESIDENT, THE ST/FED. COORDINATING OFFICER ARRANGES FOR DAMAGE SURVEYS TO BE MADE BY ST/FED. INSPECTORS & LOCAL REP.
