FROM THE DISASTER RELIEF ACT OF '74, P.L. 93-288, AND FED. DISASTER
ASSISTANCE REGULATIONS (44 CFR PART 205). FEMA POLICIES AND
GUIDELINES (P.L. 93-288/93-234) REQUIRE UNDER CERTAIN CONDITIONS
THE APPLICANT MUST OBTAIN AND MAINTAIN HAZARD INSURANCE AS A
CONDITION FOR RECEIVING DISASTER ASSISTANCE. THIS FORM DOCUMENTS
THE INSURANCE COMMITMENT AND CAN BE USED FOR FLOOD INSURANCE AS
WELL AS OTHER TYPES OF HAZARD INSURANCE
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
