Abstract: THIS FORM IS USED TO ITEMIZE THE FINAL QUANTITIES OF THE VARIOUS TYPES OF OUTSIDE PLANT FACILITIES CONSTRUCTED UNDER THE TELEPHONE SYSTEM CONSTRUCTION CONTRACT, REA FORM 515, OR UNDER FORCE ACCOUNT PROPOSAL. THE STATEMENT IS USED BY THE ENGINEER IN DETERMINING HIS ENGINEERING FEES AND BY THE BORROWER AND REA IN THEIR REVIEW AND REA IN THEIR REVIEW OF THE ENGINEER'S FINAL STATEMENT OF ENGINEERING FEES
