Abstract: THE INFORMATION (FORM) IS NEEDED TO RECEIVE BENEFITS UNDER THE SUDDEN AND SEVERE ECONOMIC DISLOCATION (SSED) PROGRAM. BECAUSE THE SSED PROGRAM RESPONDS TO UNFORESEEN DISRUPTIONS TO A ECONOMY, SPECIFIC AND NEW INFORMATION IS NEEDED TO IDENTIFY THE PROBLEM TO BE ADDRESSED.
