Abstract: THE INTEGRATED QC WORKSHEET IS USED BY HCFA, SSA, AND FNS AS THE SINGL FORM IN QUALITY CONTROL PROGRAMS. WHEN FINAL AND OPERATIONAL, IT WILL EFFECTIVELY REDUCE BURDEN BY PROVIDING FOR A SINGLE, STATISTICALLY VAL SAMPLE WHICH WILL MEET THE NEED OF ALL THREE PROGRAMS. CURRENTLY, THE FORM IS USED IN SELECTED STATES ON A DEMONSTRATION BASIS. WILL NOT BE APPLIED TO ALL STATES THIS YEAR.
