IDENTIFIES THE TAXPAYER, THE SHIPMENTS, THE AMOUNT OF TAX AND METHOD OF PAYMRNT, AND CERTIFICATION BY A PUERTO RICAN ATF OFFICER OF THE PAYMENT. FORM ALSO PROVIDES STATISTICAL INFORMATION ON LARGE CIGARS.
