Abstract: FORM IS NECESSARY FOR ANY BREWER WHO DESIRES TO CONCENTRATE OR RECONSTITUTE BEER IN THE U.S. DESCRIBES THE BREWER, PROCESS OF CONCENTRATION OR RECONSTITUTION WHERE THE BEER WILL BE SENT TO OR RECEIVED FROM AND THE DATES OF SUCH ACTIVITY. NECESSARY TO DETERMINE THE TAXABLE STATUS OF THE BEER AND ITS IDENTIFICATION FOR LABELING (CONSUMER) PURPOSES.
