Abstract: SPONSORING ORGANIZATIONS WHO WANT IRS APPROVAL OF MASTER OR PROTOTYPE PENSION PLANS THAT INCLUDE SELF-EMPLOYED INDIVIDUALS WOULD SUBMIT THIS FORM. APPROVAL MEANS THE REQUIREMENTS OF IRC SECTIONS 401(A) AND 501(A) AS WELL AS APPLICABLE PROVISIONS OF ERISA HAVE BEEN MET. THE INFORMATION REQUESTED IS NECESSARY TO DETERMINE IF THE PLAN SHOULD BE APPROVED.
