Abstract: THIS FORM IS USED TO DETERMINE THE SELECTION OF DAYS & TRIPS AND A SAMPLE OF DAYS AND TERMINALS FOR THOSE MOTOR CARRIERS DESIGNATED TO PARTICIPATE IN HIGHWAY FORM A & B COST STUDIES FOR A GIVEN YEAR.
