Abstract: THIS INSTRUMENT IS USED TO COLLECT ESSENTIAL INFORMATION FOR THE ALLOCATION OF THE FEDERAL FUNDS FOR THE TRUST FUND AND SSI PROGRAMS. IT PROVIDES RSA TIMELY INFORMATION FOR MONITORING THE VR PROGRAMS AND ANALYZING PERFORMANCE AGAINST PLANNED GOALS.
