FORMS USED BY MINERS TO REQUEST BENEFITS UNDER THE FEDERAL COAL
MINE HEALTH AND SAFETY ACT OF 1969, AS AMENDED BY THE BLACK LUNG
BENEFITS ACT OF 1972 AND THE BLBA OF 1977. THE CM-915 IS USED BY
MINERS TO REQUEST REIMBURSEMENT FOR FOR OUT OF POCKET EXPENSES
INCURRED PURCHASING DRUGS OR OXYGEN THERAPY.
