Abstract: THE FEDERAL REGULATIONS (49 CFR 195) REQUIRE LIQUID PIPELINE OPERATORS TO REPORT FAILURES TO DOT. THE FORM INCLUDES INFORMATION ON THE LOCATION OF THE FAILURE, PERSONAL INJURY AND PROPERTY DAMAGE, COMMODIT LOSS, AND CAUSE. THE INFORMATION IS USED TO EVALUATE THE EFFECTIVENES OF EXISTING REGULATIONS.
