Abstract: THIS FORM LETTER IS USED TO SECURE THE STATEMENT OF A WITNESS TO AN ACCIDENT WHEN A VETERAN HAS FILED A CLAIM FOR DISABILITY BENEFITS BASED ON INJURIES INCURRED IN THE ACCIDENT.
