THIS FORM IS JUDICIAL IN NATURE AND DESIGNED TO AFFORD APPELLANTS A "PIECE OF PAPER" UPON WHICH TO WRITE THEIR ARGUMENTS TO BE CONSIDERED BY THE APPEALS BOARD.
