IN ADDITION TO OBTAINING INFORMATION ABOUT COMPLIANCE WITH LABOR STANDARDS THIS FORM ALSO USED AS A SCREENING DEVICE TO DETERMINE WHETHER THE DIVISION HAS JURISDICTION IN HANDLING AN ALLEGED VIOLATION.
