Abstract: THE FORM IS USED TO ADVISE USERS OF CERTAIN IRS FILMS WHEN SUCH FILMS ARE TO BE RETURNED. IT ALSO REQUESTS INFORMATION ON THE NUMBER OF VIEWERS OF THE FILMS. THE SERVICE NEEDS THE INFORMATION TO DETERMINE THE EFFECTIVENESS AND AUDIENCE REACH OF THE FILMS.
