IT ELICITS THE NECESSARY INFORMATION ABOUT THE SURVIVING CHILD(REN) OF A DECEASED INSURED INDIVIDUAL. THE DATA RECEIVED ON THIS FORM IS ASSOCIATED WITH THE AGENCY'S CLAIMS DOCUMENTATION AND SUPPORTS THE PAYMENT OF MONTHLY BENEFITS TO THE SURVIVING CHILD(REN).
