Abstract: FORM IS NECESSARY TO DESCRIBE THE CONTENTS OF A PACKAGE OF SPIRITS FOR TAX PURPOSES. DESCRIBES THE DISTILLED SPIRITS PLANT THAT PACKAGES SPIRITS, TYPE AND QUANTITY OF SPIRITS IN THE PACKAGE AND PACKAGE IDENTIFICATION NUMBERS. MAY BE USED TO DETERMINE TAXES DUE ON SPIRITS LOST OR WITHDRAWN FOR ANY PURPOSE FROM A DISTILLED SPIRITS PLANT.
