Abstract: AT TIMES, A BUSINESS NOT HOLDING A PERMIT TO SECURE A TYPE OF SPECIALLY DENATURED SPIRITS MAY WISH TO OBTAIN A SAMPLE OF THE FROM A DEALER. IN SUCH CASES, THEY PREPARE THIS FORM AND UPON APPROVAL, THEY ARE ABLE TO OBTAIN THE SPIRITS. THIS FORM AND PROCEDURE CONTROL ACCESS TO SAMPLES OF SDA.
