Abstract: THE NATIONAL EYE INSTITUTE SEEKS DATA ON THE PREVALENCE OF VISUAL IMPAIRMENT IN THE GENERAL POPULATION AND ON OCULAR DISEASES PRESENT IN THE VISUALLY IMPAIRED EYES. THIS FORM WILL BE USED ON THE PERSONS WHO HAVE BEEN IDENTIFIED AS HAVING 20/40 VISION OR WORSE, AND CONTROLS, REFERRED TO THE CLINIC FOR AN OPHTHALMOLOGICAL EXAMINATION.
