Abstract: SECTION 216(H) OF THE SOCIAL SECURITY ACT PROVIDES THE NECESSARY CRITERIA TO ENTITLE ILLEGITIMATE CHILDREN TO CHILD'S BENEFITS UNDER SOCIAL SECURITY. THIS FORM IS USED TO DETERMINE THE RELATIONSHIP BETWEEN A CHILD AND THE INSURED INDIVIDUAL.
