Abstract: THIS FORM IS REQUIRED UNDER THE DEPARTMENT OF LABOR'S NEGOTIATED MERIT STAFFING PLAN FOR POSITIONS OUTSIDE THE UNION BARGAINING UNITS, NATIONAL COUNCIL OF FIELD LABOR LOCALS, AND THE NATIONAL UNION OF COMPLIANCE OFFICERS TO SOLICIT INFORMATION BY PERSONNEL OFFICES FROM THE APPLICANT'S SUPERVISOR. THE INFORMATION WILL BE USED BY RATERS TO EVALUATE OUTSIDE APPLICANTS AGAINST THE REQUIREMENTS OF THE VACANCY TO BE FILLED.
