Abstract: THIS FORM IS USED TO RECORD PACKAGES FILLED WITH DENATURED SPIRITS BY LICENSED DEALERS OF SPECIALLY DENATURED SPIRITS. THE FORM DESCRIBES THE LICENSEE, THE SIZE OF PACKAGES, QUANTITY AND TYPE OF SDS FILLED INTO EACH PACKAGE, THE PACKAGE ID NUMBER, AND DATE OF PACKAGING. FORM PROVIDES A RECORD FOR ATF TO AUDIT ACTIVITIES OF THE DEALERS.
