Abstract: THE PURPOSE OF THIS FORM IS TO NOTIFY THE DEPARTMENT OF EDUCATION OF THE NOTE TRANSFER BETWEEN THE TWO PARTIES. IT ALSO CONSTITUTES A FORMAL AGREEMENT THAT THE BUYER SUCCEEDS TO ALL RIGHTS AND RESPONSIBILITIES OF THE SELLER UNDER THE CONTRACT OF INSURANCE.
