Abstract: THE FORM COLLECTS INFORMATION ON THE SERVICES PROVIDED BY STATES IN THEIR ENERGY EXTENSION SERVICE (EES) EMPHASIS PROGRAMS. THE INFORMATION COLLECTED RELATING TO CLIENT CONTACTS IS USED TO MONITOR THE PROGRAM EFFECTIVENESS IN REACHING CLIENTS AND CARRYING OUT EES GOALS.
